Lorry made in 2023, stub axle failure suspected, report ‘incomplete’ as tyre cannot be found – Puspakom

Puspakom has submitted its findings on the tragic Ayer Keroh crash to the Malaysian Institute of Road Safety Research (MIROS) for further action, but the inspection remains incomplete as the dislodged lorry tyre that was the accident’s root cause could not be located, Bernama reports.

The investigation found that the detached tyre could have been caused by a stub axle failure on the lorry’s front axle, but inspectors are puzzled as the Foton EST 430 lorry is relatively new, having been manufactured in 2023 and inspected in March 2024, said Puspakom CEO Mahmood Razak Bahman.

“The inspection and investigation that we conducted for MIROS is incomplete even though the report was sent to the authorities today (January 2).

“This is because we could not find the tyre that was dislodged. So, Puspakom was unable to inspect the (condition of the) tyre, which should have been sent with the vehicles,” Mahmood Razak said at a press conference at Puspakom Shah Alam yesterday.

Separately, The Star reports that the two vehicle inspectors who were questioned by the Road Transport Department (JPJ) over alleged commercial vehicle inspection wrongdoings have no recent disciplinary issues.

“I have spoken to the two personnel, one with a service record of three years, and the other, 14 years. The staff member who served for three years did not have any disciplinary cases, while the other one did not have any issues in the past four years that warranted concern,” said Mahmood Razak.
